- Employment information
- Name of primary care provider
- List of medications the patient is current taking
- Bring a copy of patient’s advance directive (if available) for patient record filing purposes
- Emergency contact information
- If you are a new patient transferring from another hospital, please have your medical records sent to CRHC
- Review our Patient Privacy Practices. This information is also available in print upon request.
