Preparing for your appointment

To assist in a smooth check-in process you should bring the following items with you to your next appointment or scheduled procedure:

  1. Patient’s social security number
  2. Current insurance card
  3. Valid photo I.D.
  4. Employment information
  5. Name of primary care provider
  6. List of medications the patient is current taking
  7. Bring a copy of patient’s advance directive (if available) for patient record filing purposes
  8. Emergency contact information
  9. If you are a new patient transferring from another hospital, please have your medical records sent to CRHC
  10. Review our Patient Privacy Practices. This information is also available in print upon request.
